G Plus Lash & Brow Service Consent Form

 

I, [Client], hereby acknowledge and agree to the following terms and conditions regarding the services provided by G Plus Lash & Brow, including Eyelash Extensions, Waxing, and Semi Permanent Makeup:

 

Eyelash Extension Service:

 

I understand that eyelash extensions involve the application of synthetic lashes using adhesive, which carries a risk of allergic reactions.

G Plus Lash & Brow will not perform more than three refills after the initial lash extension set for the safety of my natural lashes.

I acknowledge that if I experience discomfort or dissatisfaction within 48 hours after the initial application, adjustments are free. After 48 hours, adjustments will incur a fee.

I also understand that any damage or issues due to the technician's technique will be adjusted for free, but damage caused by my own actions will result in a fee.

 

Waxing Service:

 

I confirm that I have voluntarily chosen to undergo waxing treatments.

I understand that proper hygiene is essential, and my esthetician may refuse service if not followed.

I declare that I have no open skin lesions or active herpes outbreaks.

I further confirm that I am not under the influence of alcohol or recreational drugs.

 

Semi Permanent Makeup Service:

 

I am over the age of 18 and desire to have semi permanent makeup performed.

I comprehend the nature, risks, and possible complications of semi permanent skin pigmentation, including infection, allergic reactions, scarring, and color variations.

I accept the semi permanence of the procedure and understand that individual reactions may affect pigment longevity.

I will follow pre- and post-procedure instructions.

By signing below, I indicate that I have read and understand the terms and conditions of this waiver, and I agree to release G Plus Lash & Brow and its staff from any liability associated with these services.
